We will be mailing the original 2007 W-2's for Old Chicago Restaurants, Rock Bottom Breweries, Chop Houses, Walnut Brewery and Sing Sings on January 30th, 2008 to your home address. If the address we have for you is incorrect, the W-2 will be returned to the restaurant and you may email us with a change of address to operations@rockbottom.com.

A free copy is available to you after Jan 13th via an affiliated Web site address. Once logged into the website, you will be instructed on the steps needed to set up your user name and password, which will enable you to print a copy of your W-2, or download it into Turbo-Tax. The User Name and Password are both the same number, which is a 10 digit number, unique to you, composed of a combination of
  (a) your 2 digit birth month
  (b) the last 4 digits of your social security number
  (c) your 2 digit birth year.
  (d) the 2 letter state code of the restaurant you LAST worked in (must be capitalized).

Example: You worked at Old Chicago in Minneapolis, Minnesota, our birthdate is June 10, 1972 and soc. security number is 222-00-3333, becomes 06333372MN. After the initial logon, the system will prompt you for a change of user name and password. Click here to access the site and get your W-2 copy. Once in the Web site, DO NOT try to create your own account - DO NOT use the "[click here] if you do not have an account". . Follow the above instructions for User Name and Password.

Or, if a copy is needed from a year prior to 2002, you may request a copy directly from our corporate office. No phone calls will be accepted. Send $5, plus a self-addressed, stamped envelope to 248 Centennial Pkwy, Louisville, CO 80027, Attn: Payroll, along with the year from which you need a copy. The charge is $5 per copy and a check should be made payable to Rock Bottom Restaurants. We will mail the copy within a week of our receipt of your request.
